Medical Expenses due to accident/sickness

Formal claim letter, copy of medical reports and treatment, copy of evidence and/or proof of expenses incurred, copy of the record of the first assistance and/or visit to the hospital, copy of studies’ results, copy of current id with photo of the insured issued by a government entity with an authority to issue (front and back), copy of travel itinerary, accident or illness claims form.

Trip Curtailment or Cancellation

Formal claim letter, copy of medical reports and evidence, proof of unreimbursed charges, copy of current id with photo of the insured issued by a government entity with an authority to issue (front and back), copy of travel itinerary, proof of purchases of the trip package, copy of birth certificate and/or marriage certificate and/or sworn statement validating domestic partnership.

Travel Delay

Formal claim letter, proof of expenses, copy of current id with photo of the insured issued by a government entity with an authority to issue (front and back), copy of travel itinerary, document from transport provider.

Lost or Damage Personal Belongings

Formal claim letter, copy of boarding pass and bag identification, copy of report of theft, damage or loss, copy of police report or complaint, copy of transport provider determination, copy of current id with photo of the insured issued by a government entity with an authority to issue (front and back), copy of travel itinerary.

Baggage Delay

Formal claim letter, proof of expenses, copy of current id with photo of the insured issued by a government entity with an authority to issue (front and back), copy of travel itinerary, airline's report and notification of baggage.
